• Please note that due to federal privacy laws regarding personal health care information, individuals are usually required to complete a MassHealth Permission to Share Information Form. This form allows the Representative’s staff to investigate health-related matters on your behalf. To expedite your inquiry, please print and fill the Form out completely and then fax it to our offices: 617-722-2821
• Do you qualify for state healthcare? Find out at the Massachusetts Health Connector website. Click on “Find Insurance,” then select who the policy holder will be by clicking on the appropriate “Shop Now” link, and click on “Start Now.”

Schedule a State House tour

• If you are coming to the State House, please let the Representative know, or stop by our office in Room 254. We are happy to arrange State House tours for large groups. Individuals may take the free State House tour, which leaves every half hour from Doric Hall.
• Free tours are available daily 10 a.m. to 4 p.m.

Request a House citation

• Representative Costello frequently sends official House citations to constituents to acknowledge special occasions, including birthdays, anniversaries, Eagle Scout Awards, and other special requests.
