This form allows the Representative’s staff to investigate health-related matters on your behalf. To expedite your inquiry, please print and fill the Form out completely and then fax it to our offices: 617-722-2821

Schedule a State House tour

  • If you are coming to the State House, please let the Representative know, or stop by our office in Room 254. We are happy to arrange State House tours for large groups. Individuals may take the free State House tour, which leaves every half hour from Doric Hall.
  • Free tours are available daily 10 a.m. to 4 p.m.

Request a House citation

  • Representative Costello frequently sends official House citations to constituents to acknowledge special occasions, including birthdays, anniversaries, Eagle Scout Awards, and other special requests.
  • Please send us an email using our Contact form, and select “Request a House citation” from the Subject options. Please be as specific as possible, and include the date by which you will need the citation.
